Services Offered: The Donated Dental Services Program (DDS), a program of the Dental Lifeline Network, provides services in private dental offices located throughout the state to those who qualify. The DDS Program is not accepting applications in all counties at this time. Eligibility: Applicants must meet the following criteria: 1.) have a permanent and severe disability, chronic illness or be elderly. The disability may be physical, mental or psychological in nature, a permanent condition and prohibits or significantly limits employment; 2.) are unable to pay for dental treatment and show that there is no other means of dental care; and 3.) need extensive treatment, more than routine care. Payment Options: Donated Care

Eligibility: Ages 13 years and up. Income no more than 200 percent poverty level. Proof of income required for everyone in the household (tax forms, pay stubs). Payment Options: Fees based on ability to pay, payment plans, Accept Healthy Michigan Plan through Delta Dental.
