Services Offered:
The Donated Dental Services Program (DDS), a program of the Dental Lifeline Network, provides services in private dental offices located throughout the state to those who qualify. The DDS Program is not accepting applications in all counties at this time.

Eligibility:
Applicants must meet the following criteria: 1.) have a permanent and severe disability, chronic illness or be elderly. The disability may be physician, mental or psychological in nature, a permanent condition and prohibits or significantly limits employment; 2.) are unable to pay for dental treatment and show that there is no other means of dental care; and 3.) need extensive treatment, more than routine care.
